Full - Time: 40 Hours Per Week2nd Shift: 1pm - 9:30pmMonday -
Friday -Hybrid: Onsite and Remote Requirements -Supports the
Psychiatric Intake Response Center and Inpatient Behavioral Health
UnitSummary:This position will serve as a coordinator of care for
patients and families, serving as the liaison for ED, inpatient,
and addiction services. This position will provide support to
identified patients and families with behavioral health needs to
triage patients in real time to the appropriate level of care
across the hospital system and assist them in navigating barriers
to treatment engagement. Responsibilities:
- In conjunction with ED, inpatient, and/ or addiction services
providers, help triage patients to appropriate level of care.
- Participate in regularly scheduled case conferences to
coordinate treatment between team members including, psychiatry,
attendings, social work, mental health therapists and other
providers.
- Assist with navigation between levels of care within Behavioral
Health.
- Work with family to arrange for behavioral health follow up
appointments and identify barriers to engagement in treatment and
engage in problem solving to address barriers.
- Conduct basic motivational interviewing interventions to assist
in addressing barriers.
- Provide education about behavioral health conditions to the
patients and families as indicated.
- Assess for social determinants of health and support
referrals.
- Coordinate/collaborate with external community partners as
needed.
- Help with patient throughput throughout the department.
- Maintain up to date community resource lists for families.
- Other duties as assigned. Other information:Technical
Expertise
- Experience working with multidisciplinary teams.
- Proficiency in Electronic Health Record computerized
documentation is preferred.
- Experience in healthcare is preferred.
- Excellent communication skills, both verbal and written,
coupled with strong listening skills.
- Strong organizational skills with the ability to multi-task, as
well as anticipate and resolve problems.
- Proficiency in MS Office (Outlook, Excel, Word, Power Point) or
similar software is required. Education and Experience
- Education: High school diploma or equivalent is required.
Bachelor's degree in related field preferred or 2-4 years relevant
experience
